What is Paradigm Senior Care Advantage?

All Medicare Advantage patients who are enrolled with Aetna, Anthem, Humana, OptumCare, SummaCare or UnitedHealthcare and have designated Pioneer physicians Network or Community Health Care as their primary care doctor of record are automatically enrolled in our Paradigm Senior Care Advantage program.

Paradigm Senior Care Advantage is a program built on the premise that through stronger connections between patients and primary care physicians; between health plans and those caring for patients; and between doctors who treat the whole person and those who specialize in a specific procedure or condition, care can be transformed.

Paradigm Senior Care Advantage is not a new Medicare Advantage plan. Rather it is a new approach to care delivery which focuses on preventative care, care coordination resources, when you need them, and an appreciation for the demands and constraints on your desired lifestyle. The primary care physicians at Pioneer Physicians Network and Community Health Care are committed to supporting you in reaching your health and wellness goals.

To participate in Paradigm Senior Care Advantage, you must be enrolled in a participating Medicare Advantage plan and designate Pioneer Physicians Network or Community Health Care as  your primary care doctor to oversee your care. 

Paradigm Senior Care Advantage by Pioneer Physicians Network and Community Health Care works in partnership with the 2023 Medicare Advantage plans listed below:

We encourage our Paradigm Senior Care Advantage patients to complete an annual wellness visit with their primary care physician. This comprehensive, individualized assessment provides an annual roadmap to improved health and wellness. It is also the gateway for our patients to access specialized programs designed for those who need more focused care coordination resources.

A licensed Medicare broker is a professional that is independent of any specific health plan, is appointed to represent multiple health plans, and can review your specific needs and help you compare Medicare coverage options for this enrollment year.
